Triglycerides Blood Test: Normal, Borderline and High Levels Explained

Triglycerides measure the fat currently circulating in your blood, and results are generally grouped as normal (under 150 mg/dL), borderline-high (150–199 mg/dL), high (200–499 mg/dL), or very high (500 mg/dL or above). A mildly elevated result after a meal is usually temporary and not concerning by itself. A persistently high fasting result is associated with increased cardiovascular risk and often reflects factors like excess weight, alcohol use, poorly controlled blood sugar, or certain medicines. Very high levels — generally 500 mg/dL and above, and especially above 1,000 mg/dL — shift the concern toward acute pancreatitis risk rather than only long-term cardiovascular risk, which is a different and more time-sensitive clinical situation.
Triglycerides are the lipid panel value most affected by what you did in the last day, not just the last decade. A single high reading after a heavy meal or a night of drinking tells you something different than a consistently high fasting level. The number that matters for long-term risk assessment is your typical baseline; the number that matters for immediate safety is how extreme a very-high reading actually is.

What the test measures

Because chylomicrons appear in blood specifically in response to recent eating, a triglyceride level drawn shortly after a meal reflects both your baseline and your last meal combined.
How the main pattern is interpreted
Interpretation depends heavily on context. A borderline-high reading in someone who ate within the last few hours is very different from the same reading fasted. A persistently high fasting triglyceride level, especially alongside low HDL and high LDL, is read as part of an atherogenic pattern often connected to insulin resistance or metabolic syndrome. A very high level — particularly above 1,000 mg/dL — is read through a different lens entirely: the immediate concern becomes chylomicronemia and acute pancreatitis risk rather than long-term atherosclerosis.
Reference ranges versus cardiovascular-risk thresholds
The NCEP ATP III categories — normal, borderline-high, high, and very high — function as both a reference framework and a rough risk indicator, but they’re not identical to a treatment target. The Endocrine Society’s 2012 clinical practice guideline adds finer detail at the top end: triglycerides of 1,000–1,999 mg/dL are categorized as "severe" hypertriglyceridemia, and levels above 2,000 mg/dL as "very severe," both associated with meaningfully higher pancreatitis risk than the "very high" category alone conveys. It’s worth being precise here: 500 mg/dL is the point where pancreatitis risk starts entering the conversation, but the risk becomes substantially more pressing as levels climb well beyond that, particularly past 1,000 mg/dL.
What can distort the result

Alcohol consumed in the prior day or two can meaningfully raise triglycerides. Poorly controlled blood sugar, whether from undiagnosed or poorly managed diabetes, is a strong driver of elevated triglycerides. Hypothyroidism can contribute to elevated triglycerides in some people. Certain medicines — including some estrogen-containing therapies, corticosteroids, and certain diuretics — can raise triglycerides as a side effect. Obesity and excess abdominal fat are commonly associated with chronically elevated levels.
What the result does not prove
An elevated triglyceride result does not, by itself, identify which of these causes is responsible, and it does not confirm that pancreatitis will occur even at very high levels — it indicates elevated risk, not a certainty. It also doesn’t distinguish a temporary post-meal spike from a genuine metabolic pattern without a fasting repeat or a clearer clinical picture.
Common misunderstandings
A common misunderstanding is treating any triglyceride reading above 500 mg/dL as an immediate medical emergency in every case — the risk genuinely escalates with the level, and levels well above 1,000 mg/dL carry meaningfully more urgency than levels just over 500. The opposite misunderstanding — dismissing a borderline-high fasting result as unimportant because "everyone’s triglycerides go up sometimes" — overlooks that a persistently elevated fasting baseline is genuinely associated with cardiovascular risk, distinct from a one-off post-meal reading.
