HbA1c Test: Normal Range, Prediabetic, Diabetic Cut-offs & How to Lower It
HbA1c reflects your average blood sugar over ~3 months. Understand every threshold — normal, prediabetes, diabetes — and the realistic actions that move the number.

HbA1c below 5.7% is normal, 5.7–6.4% is prediabetes, and 6.5% or higher on two occasions confirms diabetes. Each 1% rise in HbA1c corresponds to roughly a 28.7 mg/dL rise in average blood glucose. Diet quality, weight, exercise and sleep move the number more than most patients expect.
HbA1c measures the fraction of haemoglobin that has irreversibly bound glucose. Because red blood cells live ~120 days, HbA1c reflects the average blood glucose across the past 8–12 weeks. ADA and WHO thresholds: normal < 5.7% (< 39 mmol/mol), prediabetes 5.7–6.4% (39–47 mmol/mol), diabetes ≥ 6.5% (≥ 48 mmol/mol). Two abnormal results — or one abnormal HbA1c plus classic symptoms — confirms the diagnosis. Anaemia, haemoglobinopathies, and rapid red-cell turnover distort the reading; verify with fasting glucose or an OGTT if in doubt.

What HbA1c really measures
When glucose sits in the bloodstream it slowly and irreversibly attaches to haemoglobin, the oxygen-carrying protein inside red blood cells. HbA1c is the percentage of haemoglobin that has been glycated this way. Because red blood cells live for about 120 days, the HbA1c you measure today is a weighted average of the last 8–12 weeks of blood glucose — with the most recent 30 days weighted most heavily.
This is why HbA1c does not need a fasting sample and why a single high reading is more meaningful than a single high finger-stick glucose. It captures the "average" a snapshot cannot. It is not, however, a substitute for a glucose meter when you are titrating insulin — HbA1c cannot see night-time hypoglycaemia or post-meal spikes.

How to read your result
The single most important step is repeating any abnormal HbA1c before accepting a diabetes diagnosis. Guidelines require either two abnormal tests (any combination of HbA1c, fasting glucose or 2-hour OGTT) OR one abnormal test plus classical symptoms — polyuria, polydipsia, blurred vision, unintentional weight loss.
For patients already diagnosed, most adults with type 2 diabetes have a treatment target of < 7% (< 53 mmol/mol). Individualisation matters: a healthy 45-year-old newly diagnosed may aim for < 6.5%; a frail 82-year-old with hypoglycaemia-prone diabetes may safely sit at 7.5–8%. Tighter is not always better.
When HbA1c is misleading
HbA1c relies on normal red cell lifespan. Anything that shortens or lengthens that lifespan distorts the number. Iron deficiency anaemia can nudge HbA1c up by 0.3–0.5% (older cells accumulate more glucose). Recovery from significant blood loss, haemolysis, sickle-cell trait, thalassaemia, and recent transfusion push it artificially low. Pregnancy shifts it down; chronic kidney disease and iron overload shift it up.

What actually lowers HbA1c
Six interventions have the strongest evidence per unit of effort: (1) losing 5–10% of body weight — moves HbA1c down 0.5–1.5% in prediabetes; (2) 150 minutes of moderate-intensity aerobic activity per week — 0.5–0.7% drop; (3) resistance training 2–3× per week — additive to aerobic exercise; (4) reducing refined carbs and switching to a Mediterranean or low-carb pattern — 0.5–1.2% drop; (5) improving sleep (7–9h) — hypnotic sleep restriction alone raises fasting glucose within a week; (6) metformin, semaglutide or SGLT2 inhibitors when lifestyle alone is insufficient — 0.8–1.8% drop depending on baseline.
Combinations compound. Diabetes Prevention Program participants who lost 7% body weight and exercised 150 minutes per week cut diabetes progression by 58% over 3 years — a bigger effect than any single medication.
