Ammonia Blood Test: When Elevated Levels Matter (and When They Do Not)
Ammonia is measured mostly to investigate hepatic encephalopathy and inherited urea cycle disorders. Learn why a single high ammonia is often unreliable and what patterns actually matter.

Normal ammonia is roughly < 35 µmol/L in adults. Ammonia is measured mainly to investigate suspected hepatic encephalopathy in cirrhosis, urea cycle disorders in unexplained coma, and valproate toxicity. Sample handling errors produce many falsely high values — use ice-packed samples processed within 15 minutes.
Ammonia is produced by gut bacteria and by amino acid metabolism, then cleared by the liver via the urea cycle. Cirrhosis, portosystemic shunting, urea cycle defects, valproate, and salicylate toxicity impair clearance. Ammonia contributes to hepatic encephalopathy (HE) but correlation with severity is imperfect — clinical grading (West Haven) is more reliable than ammonia level. Guidelines (AASLD, EASL) do NOT recommend ammonia measurement for diagnosis or monitoring of HE in typical cirrhotic patients; it is helpful in atypical or paediatric cases. Falsely elevated results are common — sample must be arterial or venous without tourniquet, on ice, processed within 15 minutes.

Why ammonia levels can mislead
The correlation between ammonia and encephalopathy severity is poor. Two patients with the same ammonia can have very different mental status. Sample handling has an outsized influence — a delayed or unrefrigerated sample can double the value. AASLD and EASL guidelines therefore reserve ammonia measurement for cases where HE diagnosis is uncertain, paediatric coma workup (urea cycle disorders), and valproate toxicity investigation.

Treatment of hepatic encephalopathy
Lactulose reduces ammonia absorption in the colon; rifaximin reduces ammonia-producing gut bacteria. Precipitants such as GI bleeding, infection, constipation, electrolyte disturbance, sedatives, and diuretic excess should be identified and corrected.
