Naegele’s rule, briefly

Franz Naegele (1830) observed that most pregnancies delivered 280 days (40 weeks) after the first day of the last menstrual period. The rule assumes a 28-day cycle with ovulation on day 14; longer cycles push the due date later by the extra days. Ultrasound dating in the first trimester is more accurate when cycles are irregular or LMP is uncertain.

When the scan overrides the LMP

ACOG 2017 recommends using first-trimester ultrasound dating (crown-rump length between 8 and 14 weeks) as the reference if it differs from LMP-based dating by more than 5-7 days. Later scans are less accurate for dating.

Foetal movement matters

After 24 weeks, awareness of your baby’s movement patterns is one of the most important self-monitoring habits. If you notice a persistent change or reduction in movements, contact your maternity team promptly — do not wait until the next appointment.

A due date is not a deadline

Only about 4-5% of pregnancies deliver exactly on the estimated date. Most deliver between 37 and 42 weeks. Your maternity team monitors closely after 41 weeks and usually discusses induction options between 41-42 weeks.
