How the percentile is calculated

CDC 2000 growth reference uses the LMS method (Cole & Green 1992): three age-and-sex-specific parameters (L, M, S) allow computation of a z-score for any BMI value, then a percentile from the standard normal distribution. This tool interpolates between yearly waypoints — clinical software uses monthly resolution which is slightly more precise; a paediatrician has access to the more granular tables.

CDC 2000 category bands

Under 5th percentile: underweight. 5th to 85th: healthy weight. 85th to 95th: overweight. 95th or above: obesity. These are the reference bands used in AAP 2023 and most international paediatric guidance.

Trajectory matters more than a single point

Children move between percentile bands naturally as they grow, particularly around puberty. What matters clinically is the trajectory over time — a child who has tracked at the 20th percentile all their life is different from one who was at the 80th and dropped to the 20th.
